Person-Centered Care with COMS

COMS’ Care Guides
What are Care Guides? Care Guides are specific to COMS. They contain years of research and data and serve as disease-specific tools that drive the clinical engines that feed the COMS assessments and Care Review process. These Care Guides are dynamically applied based on the Resident’s specific disease profile. Person-centered care.

Care Guides are important for many reasons. The two main reasons:
1. Clinical Process – Nurses are over-burdened as it is. The expectations of them are high. They need standardization and process to provide care while managing multiple comorbidities. COMS Clinical Process provides the foundation for your Nurses to be efficient and successful in their care delivery. Care Guides are the foundation of that process.
2. Driving Documentation – The Care Guides contain everything necessary to provide complete care. The result of good clinical care is better documentation. Not only do the Care Guides within the assessment drive clinical care, they also assure that your documentation reflects all of the relevant care related to that resident’s specific needs.
